Location

• Employment Type: LTA (Long-Term Assignment)
• Contract Length: 12 weeks
• Work Schedule:

  • 40 hours per week
  • Options: 8x5 or 4x10 (whichever the healthcare professional prefers)
  • Weekends: Every 4th weekend
  • No call required

Job Description

The position requires all certifications to be completed by the time of submission.

Required Credentials and Certifications

1. License/Certificate Verification

   • Primary source verification of professional license and/or certification required within 30 days of start.

2. COVID-19 Vaccine

   • Proof of COVID vaccine series or bivalent booster required. Medical exemptions must be approved prior to start.

3. MMR (Measles, Mumps, Rubella)

   • Proof of 2 immunizations or positive titer required.

4. Fit Test

   • N95 mask fit testing required prior to start.

5. Hepatitis B

   • Complete vaccination series or declination required.

6. Influenza Vaccine

   • Documentation required from September to May.

7. Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire

   • Required if history of positive PPD/TB blood test.

8. Driver's License/State ID

   • Copy required.

9. 2 Step PPD OR IGRA (Quantiferon/Tspot)

   • Must be completed within the last year.

10. Varicella

    • Proof of 2 immunizations or positive titer required.

11. Tdap

    • Proof of immunization within the last 10 years required.

12. CNE Profile

    • Resume or profile submission required.

13. CNE 2 Professional References

    • Minimum of 1 supervisory reference required.

14. CNE Criminal Background Check Results

    • Federal, County, National, and Statewide background check required within 6 months of start.

15. CNE First Look Orientation Packet

    • All testing and forms must be signed and uploaded for review prior to start.

16. CNE Non-Employee Confidentiality Agreement

    • Must be signed before start.

17. CNE Employment Verification

    • Verification of the last 5 years required.

18. CNE Covid and Flu Policy CNE Form

    • Must be completed annually.

19. CNE Confirmation of Assignment

    • Required prior to start.

20. CNE Work Attestation

    • Form and instructions available in the document library.

21. CNE Badge Photo

    • Neutral colored background, forward-facing photo required.

22. CNE Occ Health Request for Clearance Form

    • Required upon acceptance of offer.
