Job Description

The Speech Language Pathologist will provide speech therapy services to patients in their homes, addressing impairments, functional limitations, or disabilities resulting from various causes. The patient demographic ranges from infants to seniors over 100 years old.

Key Responsibilities

• Conduct age-appropriate evaluations reflecting patient history and relevant tests.
• Reassess treatment plans every two weeks and modify as necessary.
• Ensure physician orders are current and consistent with the plan of care.
• Manage patient schedules and caseloads in accordance with agency and state guidelines.
• Complete documentation timely and according to agency standards.
• Collaborate with patients, families, physicians, and other health professionals to coordinate services.

Qualifications

• Experience: Minimum of two years in acute care, rehab, or homecare as a Speech Therapist.
• Licensure: Must be registered with the Board of Registration in Massachusetts.
• Skills: Proficient computer skills, strong communication abilities, and excellent interpersonal skills.

Required Credentials

• BLS Certification: AHA, Red Cross, or equivalent.
• License Verification: Primary source verification of professional license within 30 days of start date.
• COVID-19 Vaccination: Completed primary series or up-to-date vaccination required.
• TB Test: Documentation of TB skin test screening or negative QuantiFERON TB Test within 3 months of start date.
• Health Statement: Signed by a medical professional within 365 days of start date.
• Background Checks: Various checks including criminal background, education verification, and exclusion checks.

Additional Information

• Weekend rotations are required once every 6 weeks.
• Documentation and compliance with agency standards are critical for this role.
• Strong preference for candidates with experience in home health settings.
