Occupational Therapist - Home Health (Travel/Contract)
Location: Dover, Delaware

Job Description
- Provide Occupational Therapy services in a Home Health setting with home visits and periodic office meetings.
- Average caseload: 5-6 patients per day.
- Patient visits include Start of Care and Follow Ups.
- Must be willing to travel up to 75 miles from the home clinic, primarily covering Kent County.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM.
- No weekend rotation or on-call required for this position.
- Productivity expectation: minimum of 30 points per week (points assigned based on visit type, documentation, travel, etc.).
- EMR system used: HCHB (Homecare Homebase).
- Must have recent Home Health experience; first-time home health therapists are not accepted.
Requirements
- Minimum 1 full year of Adult Home Health experience required.
- Current Occupational Therapy license in hand (pending licenses generally not accepted unless only a few days to obtain).
- CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Reliable transportation and current driver's license required.
- Agency-required liability insurance must be maintained.
- Must be willing to comply with all facility and state-specific compliance and documentation requirements.
Travel & Logistics
- Mileage reimbursed at IRS standard rate.
- Compensation covers travel from patient home to patient home; commute to/from first and last patient is not billable.
- Local candidates accepted at the same rate.
Compliance & Documentation
Candidates must complete and maintain the following (prior to start and annually if working more than one year consecutively):
- Background checks including 7-year history, sex offender registry, state-specific searches, and fingerprinting as required by state and facility.
- Health documents including physical exam, TB test or questionnaire, immunizations (MMR, Hepatitis B, Varicella, Flu, COVID-19), and drug screening.
- Facility-specific documents such as confidentiality forms, compliance training, HIPAA and privacy policies, home health scenarios, and code of conduct.
- State-specific and facility-specific authorizations and releases as applicable.
Additional Notes
- The position is workload-driven; patient census may vary. If hours are insufficient, contractors may be floated to nearby clinics to pick up additional hours.
- Candidates must be prepared to sign new supplier packets at the start of each assignment and after any break in service.
- Some positions may require additional state-specific or facility-specific requirements depending on the territory.
