Home Health – Physical Therapist (Days)
Start Date: May 19
Location: Palm Desert, CA (and surrounding areas)
Contract Length: 13 Weeks
Schedule: Full-time, 40 hours/week

We are currently seeking an experienced and dedicated Physical Therapist for a Home Health assignment. This position is ideal for a PT who thrives in a flexible, patient-centered environment. Candidates must live within 30 minutes of their assigned territory. If traveling, housing must be arranged nearby.

Key Responsibilities:

• Deliver in-home physical therapy services to a caseload of approximately 20–25 patients per week
• Complete thorough, accurate, and timely documentation using Homecare Homebase (HCHB) and OASIS E
• Provide care based on individualized plans, patient goals, and physician orders
• Attend weekly in-office meetings (typically Tuesday–Thursday)
• Maintain a minimum average of 26–28 productivity points per week (equivalent to 5–7 points per day)
• Ensure same-day documentation is completed during patient visits
• Collaborate effectively with interdisciplinary team members
• Float to nearby territories as needed

Requirements:

• Active California Physical Therapy license
• BLS certification
• Minimum 2 years of physical therapy experience
• Will Train, Home Health experience NOT required
• No employment gap greater than one year
• Must have reliable transportation

Additional Details:

• No on-call requirement
• Weekend and holiday shifts may be required (refer to assignment specifics)
• All requested time off (RTO) must be submitted in advance and approved
• Must be willing to float if needed
• Scrub color: Any
• PPE and parking details to be determined
• Orientation includes 2–3 days of onboarding and team introduction
