What You’ll Do:

As a Home Health PT, you’ll play a critical role in helping clients improve mobility, safety, and independence at home. Responsibilities include:

Making home visits in assigned territories
Conducting physical assessments and diagnostic evaluations
Designing and implementing individualized care plans
Educating patients and caregivers to support continued progress
Documenting all care using a user-friendly EMR platform

What You Need:

Active Physical Therapist license in Pennsylvania
Minimum of 1 year of recent clinical experience as a PT
Strong interpersonal and time management skills
Graduation from an accredited Physical Therapy program
