Shift: 9:00 AM – 5:00 PM | 35 hours/week

Position Summary

We are seeking a Licensed Master Social Worker (LMSW) to provide behavioral health support in an acute care setting. The LMSW will be responsible for conducting psychosocial assessments, coordinating care with the interdisciplinary team, and assisting with discharge planning. Hospital experience is strongly preferred.

Key Responsibilities

Complete comprehensive psychosocial assessments
Develop and implement discharge plans in coordination with clinical staff
Provide emotional support and crisis intervention for patients and families
Refer patients to community resources, including mental health and social support services
Maintain accurate and timely documentation in compliance with policy and regulatory standards

Requirements

Valid NYS LMSW license (Primary Source Verified)
Minimum 1 year of LMSW experience
Hospital or acute care experience preferred
Strong interpersonal, documentation, and advocacy skills
