Position Details

• Title: Market Director, Case Management
• Employment Type: Interim Leader
• Contract Length: 13 weeks
• Department: Case Management
• Reports To: Market CFO, partners with Chief Medical Officer (CMO)
• Reason for Interim: Immediate impact needed within 30 days
• Weekend Hours: Every other weekend
• Call Requirements: No formal call; available as needed for department support

Required Qualifications & Experience

• Registered Nurse (RN) license required
• AHA Basic Life Support (BLS) certification required
• Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred
• Director-level experience in a hospital with >150 beds
• Experience managing large teams (~20 staff)
• Experience across Acute Care, Long-Term Acute Care (LTAC), Skilled Nursing Facilities (SNFs), and Transitional Care Units (TCU)
• Must have EPIC electronic health record experience
• Knowledge of InterQual criteria, discharge planning, Conditions of Participation, and CMS guidelines
• Ability to provide education and guidance to a younger provider group to improve discharge processes and confidence

Key Responsibilities

• Lead Case Management across the health system with primary duties at Conemaugh Memorial
• Maintain full census in Transitional Care Unit (TCU)
• Oversee utilization review (UR) as part of Case Management with opportunities for improvement
• Implement immediate changes to improve key metrics such as length of stay, readmission rates, denial rates, and discharge planning
• Provide mentorship and foster a positive, collaborative team environment
• Utilize KPIs and technology to drive performance improvement
• Facilitate monthly executive reviews with hospital and division leadership
