Job Description

• Position: Registered Nurse for Home Health in Southern California (Riverside, South Temecula area)
• Schedule: Varied days, either Tuesday-Saturday or Wednesday-Sunday
• Experience: Minimum 1 year of home health experience, specifically with geriatric patients
• Certifications: BLS/CPR required; CA nursing license preferred (pending CA license accepted if applied for)
• Required Skills: Experience with OASIS documentation
• Hours: Based on completed patient visits (25 visits = 40 hours); hours guaranteed if 25+ visits completed
• COVID-19: Exemptions permitted
• Travel: Mileage reimbursed at IRS standard rates between patients

Required Certifications & Licenses

• BLS (Basic Life Support) - AHA Instructor or Provider acceptable
• ACLS (Advanced Cardiac Life Support) - AHA Instructor or Provider acceptable, as applicable
• PALS (Pediatric Advanced Life Support) - as applicable
• NRP (Neonatal Resuscitation Procedure) - as applicable
• EFM (Electronic Fetal Monitoring) - as applicable
• State Nursing License (Primary source verification required)
• NURSYS Verification for RN, LPN/LVN, or AP

Required Documents & Compliance

• Government-issued photo ID (Driver’s License or State ID)
• Valid Passport copy before start
• Photo for badge (passport style, shoulders up, front facing, plain background)
• Personal or auto insurance documentation (must be current during assignment)
• Auto registration (must be current during assignment)
• Signed acknowledgments for Neogen Care training content and hand hygiene policy

Health & Immunization Requirements

• Physical exam within 12 months of start date
• TDAP vaccine within the past 10 years (no declinations accepted)
• Negative TB test (PPD, QGold, or TSpot) within 1 year; if past positive, annual TBQ and chest X-ray within 5 years required
• Proof of immunity or vaccination for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Varicella (Chickenpox)
• Hepatitis B titer or vaccine series (declination accepted if within 1 year)
• Negative 9-panel drug screen within 1 year (instant drug screen acceptable)
• Seasonal flu vaccination/documentation required year-round (declinations accepted)
• COVID-19 vaccination or approved medical/religious exemption required prior to start, including booster dose

Additional Requirements

• Background check updated annually (7-year search including SS trace, county searches, nationwide sex offender check)
• EPLS/GSA/SAM and HHS/OIG checks required annually
• Two professional references required
• Annual skills checklist required
• Medication test/pharmacology competency exam with minimum passing score of 80% (or Angoff score if higher)
• Competency exam as applicable to profession
