Job Details
- Start Date: ASAP
- Shift: 8-hour day shifts only (no 4x10 or 3x12 shifts)
- Patient Ratio: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
- Experience Required: Minimum 1 year of skilled home health nursing experience
- First-Time Travelers: Not accepted
- Weekend Requirement: No weekends required
- Certifications Required: BCLS (Basic Cardiac Life Support)
- On-Call: Not required
- Floating: No floating between units required
- Block Schedule: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius
- Pending License: Accepted (confirm restrictions with hiring manager)
- Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)

Required Certifications & Verifications
- BCLS (Basic Life Support) - AHA or Canadian Heart & Stroke Foundation certification
- NURSYS license verification for RNs
- State nursing license (Oregon) - pending licenses accepted with confirmation
- Background check within 45 days of start, including SSN trace, death master index, nationwide and county checks, MVR (required for home health), and Oregon Abuse/Neglect search if applicable
- Drug screening: 10-panel within 90 days of start (no instant tests)
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health requirements:
- TDAP within 10 years (declination accepted)
- Negative TB test or chest x-ray within 12 months (annual updates required)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine during flu season (declination accepted)
- COVID-19 vaccination (primary series plus booster required)
- Respirator fit testing (annual, travel only)
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty within the last 3 years
Training and Compliance Modules
- Completion of Providence Health and Services WorkRamp modules prior to start, including:
- Core modules for Oregon RN/LPN Home Health
- Regulatory and annual compliance modules
- EPIC training (if applicable)
- OASIS documentation training
- Modules assigned and tracked through Aya's LMS platform (WorkRamp)
Additional Notes
- Float policy acknowledgment required (may be asked to float to other Providence facilities within reasonable distance)
- Facility requires annual updates for physical exam, TB test, respirator clearance, and background checks
- Locals within 50-75 miles considered local and paid at the same rate as travelers
- No weekend or on-call shifts required
