Start Date: ASAP
Shift: 8-hour day shifts only (no 4x10 or 3x12 shifts)
Patient Ratio: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
Experience Required: Minimum 1 year of skilled home health nursing experience
First-Time Travelers: Not accepted
Weekend Requirement: No weekends required
Certifications Required: BCLS (Basic Cardiac Life Support)
On-Call: Not required
Floating: No floating between units required
Block Schedule: Not accommodated
Locals Accepted: Yes, within 50-75 mile radius
Pending License: Accepted (confirm restrictions with hiring manager)
Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)

Client grouping minimizes drive time; occasional travel radius up to 50+ miles

Required Certifications & Verifications

State nursing license (Oregon) - pending licenses accepted with confirmation
Background check within 45 days of start, including SSN trace, death master index, nationwide and county checks, MVR (required for home health), and Oregon Abuse/Neglect search if applicable
Drug screening: 10-panel within 90 days of start (no instant tests)
Physical exam within 1 year of start, signed by MD, NP, PA, or DO
Immunizations and health requirements:
- TDAP within 10 years (declination accepted)
- Negative TB test or chest x-ray within 12 months (annual updates required)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine during flu season (declination accepted)
- COVID-19 vaccination (primary series plus booster required)
Respirator fit testing (annual, travel only)
References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty within the last 3 years

Training and Compliance Modules

Completion of Providence Health and Services WorkRamp modules prior to start, including:
- Core modules for Oregon RN/LPN Home Health
- Regulatory and annual compliance modules
- EPIC training (if applicable)
- OASIS documentation training
Modules assigned and tracked through Aya's LMS platform (WorkRamp)

Additional Notes

Float policy acknowledgment required (may be asked to float to other Providence facilities within reasonable distance)
Facility requires annual updates for physical exam, TB test, respirator clearance, and background checks
Locals within 50-75 miles considered local and paid at the same rate as travelers
No weekend or on-call shifts required
