Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits with an average of 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, current driver's license, and agency-required liability insurance.
• Willingness to travel up to 75 miles from the home clinic; approximately 200 miles traveled weekly.
• Use of HomeCare HomeBase (HCHB) EMR system required; proficiency in Oasis is also required.
• Schedule typically Monday-Friday, 8 AM to 5 PM; some positions may have a Baylor shift schedule (4x10s, Friday-Monday).
• On-call required approximately 2 times per month.
• No guaranteed hours; if patient census is low, may be floated to a nearby clinic.
• Weekend requirements vary by location and should be discussed during the interview.

Requirements

• Minimum 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
• First-time travelers not accepted; recent specialty experience required.
• Current Registered Nurse (RN) license in hand; pending licenses not accepted.
• CPR and BLS certification required.
• Proficiency with HCHB within the last 6 months; retraining required if last use was over 6 months ago.
• Must be willing to comply with agency policies and complete required documentation and training.

Work Environment & Expectations

• Dress code: Scrubs.
• Productivity expectation: 30 points per week (points assigned based on visits and activities).
• Reporting location: 12021 NE Glenn Widing Drive Suite G, Portland, OR 97220.
• Workload due to leaves of absence and increased patient census.

Compliance & Documentation Requirements

Candidates must complete and maintain the following prior to and during assignment:

Certifications & Training

• BLS (Basic Life Support) current certification.
• OASIS E course if not proficient with HCHB in the last 6 months.
• Compliance training, code of conduct, privacy policies, HIPAA, confidentiality agreements, and COVID-19 policy attestation.
• Home Health Core Policy and Scenario reviews.

Licensure & Background

• Valid RN license for Oregon.
• Standard background checks including 7-year history, sex offender registry, and state-specific searches.
• Fingerprinting as required by state and facility policies.
• Drug screening (10-panel).
• Verification of education and work history (last 2 years).
• References as required by state regulations.

Health & Immunizations

• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months or completion of TB questionnaire if testing not possible.
• Proof of immunizations or titers for Measles (Rubeola), Mumps, Rubella, Varicella, Hepatitis B.
• Seasonal flu vaccination or declination.
• COVID-19 vaccination series completion or declination.

Insurance & Transportation

• Personal or auto insurance required for home health placements; policy must be current and under candidate’s name or list candidate as a driver.
• Reliable transportation required for travel to patient homes.

Additional Notes

• Local candidates are accepted at the same rate.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not billable).
• On-call and weekend requirements vary; discuss during interview.
• Must be able to complete all required documentation and training prior to start.

This position is ideal for experienced Registered Nurses with recent home health experience who are comfortable with travel within the Portland metro area and working independently in a home care setting.
