Job Description

The Registered Nurse (RN) in Home Health is responsible for coordinating and directing home nursing care according to physician's orders, agency policies, and procedures. Key responsibilities include:

• Application of the nursing process in patient care.
• Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conducting home visits and participating in periodic office meetings.

Patient Care

• Average Number of Patients per Day: 5-8
• Types of Cases: Start of Care, Wound Care, Follow-up Visits, etc.

Experience Requirements

• Years of Experience Required: Minimum of 1 full year of Adult Home Health Experience or 1 full year of Hospice experience.
• First-timers Accepted: No, must have recent specialty experience.

Schedule and Travel

• Contract Length: 13 weeks
• Schedule: M-F, 8 AM - 5 PM
• Weekend Requirements: Varies by location and profession.
• On-call Requirements: Varies by location and profession.
• Travel Radius: Must be willing to travel up to 75 miles from the home clinic.

Certifications Required

• CPR
• BLS

Additional Information

• Charting System: Proficiency in HomeCare HomeBase (HCHB) is required, with recent experience preferred.
• Dress Code: Scrubs
• Mileage Reimbursement: IRS Standard rate for travel between patient homes.

Compliance and Documentation

• Background checks
