Employment Details

• Contract Length: 12 months
• Schedule: 40 hours per week; 8:00 AM - 4:30 PM

Required Credentials and Certifications

1. BLS Certification

   • Accepted from AHA, Red Cross, CHSF, or Military.
   • Required for onboarding.

2. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification.
   • Must be completed within 30 days of start date.
   • MA Conditional Licenses not accepted.

3. Fit Test

   • N95 fit test required within 365 days prior to start date.
   • Must be completed on the MGB N95 Respirator Fit Test Record Form.

4. COVID-19 Vaccine

   • Completed primary vaccination series required.
   • No exemptions or waivers considered.

5. MMR Vaccination

   • Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity via blood test.

6. Influenza Vaccine

   • Required during flu season (September 15 - April 30).
   • No exemptions or waivers considered.

7. TB Test

   • Documentation of TB skin test or negative QuantiFERON TB Test within 3 months of start date.

8. Health Statement

   • Signed by MD/DO/NP/PA within 365 days of start date.

9. MGB Criminal Background Check

   • Must be completed within 45 days prior to start date.

10. MGB Massachusetts CORI

    • Must be completed within 30 days prior to start date.

11. MGB Resume

    • Include past 7 years of work experience and education.

12. MGB Skills Checklist

    • Required for submission.

13. MGB Confirmation of Assignment/POI Form

    • Required for all initial assignments and extensions.

14. Education Verification

    • Must verify highest level of education achieved.

15. MGB National Sex Offender Check

    • Conducted within 30 days of start and annually.

16. MGB Exclusion Checks

    • Conducted within 30 days of start and annually.

Additional Information

• Preferred Credentials:

  • Hepatitis B vaccination record or proof of immunity.
  • Tdap vaccination within 10 years.

• References:

  • Two recent references required, with at least one from a supervisor or above.

• Submission Requirements:

  • All required documents must be uploaded as PDFs.
  • Attestation forms and checklists must be completed and submitted.
