Required Credentials and Certifications

Personal Health History

• COVID-19 Vaccine: Proof of vaccine and 1 booster or exemptions must be submitted for approval.
• MMR: Proof of 2 immunizations, positive IGG titer; if negative titer, must have booster or Declination accepted.
• Hepatitis B: Proof of 3 immunizations and positive antibody titer; if negative titer, booster or Declination accepted.
• TB Test: Must be within 30 days of start. Negative TB skin test, QuantiFERON Gold, or T-spot required. Must state negative results.
• Health Statement: Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
• Varicella: Proof of 2 immunizations, positive titer, or Declination accepted.
• Tdap: Proof of vaccination within the past 10 years or Declination accepted.
• Drug Screen: Must be within 30 days of start, 9 panel minus THC. Updated annually.

Professional Credentials

• Driver's License/State ID: Required.
• Resume or Profile: Required for submission.
• Professional References: 2 required, with at least 1 supervisory reference.
• Criminal Background Check: Must be within 30 days prior to start. Updated every 3 years thereafter.
• OIG Search: Manual search required within 30 days of start and updated annually.
• EPLS: Manual search required within 30 days of start and updated annually.
• OFAC: Manual search required within 30 days of start and updated annually.
• National Sex Offender Search: Manual search required within 30 days of start and updated annually.
• Medi-Cal Suspended and Ineligible Provider List: Must be completed within 30 days prior to the first assignment and updated with each extension.

Certifications

• BLS: AHA or ARC only.
• ACLS: AHA or ARC only.
• NRP: Must be completed through AAP NRP learning platform.
• AWHONN/Fetal Heart Monitoring: Required once every 2 years.

Education Verification

• Education Verification: A diploma, degree, or transcripts is acceptable. Primary source verification of highest level of education required prior to first assignment and upon requalification.

Additional Requirements

• Skills Checklist: Clinical only; skill-specific required to be completed within 1 year of start.
• HLC Confirmation: Testing to be completed by the Friday before start.
• Confidentiality of Receipt: Must be signed and dated by the candidate.

Additional Information

• Influenza Vaccine: Required from October - March unless otherwise indicated. Medical/Religious exemptions will be sent for approval with appropriate documentation.
