Job Description
- Start Date: ASAP
- Guaranteed Hours: 24 hours per week
- Shift Requirements:
- Monday to Friday, 8:00 AM - 4:30 PM
- Every 7th weekend rotation
- On Call/Call Back Requirements: Not applicable
Qualifications
- Required Certifications:
- Master’s Degree in Social Work (MSW)
- Wisconsin License
- Minimum Experience:
- 1 year of experience in social services and counseling in a hospital acute care setting with a diverse population
- Charting Experience: Epic
Unit Information
- Unit Type: Med Surg and Critical Care Clusters (all inpatient units)
- Patient Ratio: 8-20 patients per day based on acuity
- Work Model: Layered Model working in tandem with RN Team for Discharge Planning
- Scrub Colors: Business Attire
