Job Details

Shift/Schedule: Monday to Friday (flexible on some occasions)
Weekly Point Expectations: 7-8 points
Charting System: Perfect Notefield (training provided)
Patient Population: Primarily geriatric
Experience Required:
- Prefer 1 year of Occupational Therapy experience
- Open to new graduates and those without home health experience
Interview: Phone interview

Required Certifications & Licenses

ACLS (Advanced Cardiac Life Support) – American Heart Association or American Red Cross
BLS (Basic Life Support) – preferably American Heart Association or American Red Cross
PALS (Pediatric Advanced Life Support) – preferred
State License verification (primary source online verification)
National Tech Certification (if applicable)

Required Competencies

Medication Test/Pharmacology Competency Exam (as applicable)
Competency Exam (as applicable)

Health & Immunization Requirements

Physical exam within 1 year of start
Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year or negative chest X-ray if past positive TB
Immunizations or proof of immunity for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
Seasonal Flu Vaccination or Declination (required during flu season: Oct 1 - Mar 31)
9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)

Background & Compliance Checks

Standard background check updated annually (7-year search including SS trace, county searches, nationwide, sex offender registry)
EPLS/GSA/SAM checks (annual)
HHS/OIG checks (annual)
2 Professional References required for travel, allied, per diem acute care roles (within 3 years of start)
Skills Checklist (annual)
